Patient Navigation Center (ACS) assists the cancer patient from diagnosis through treatment and long-term survival by identifying and accessing resources and support for the patient and their family.

www.cancer.org offers statistics, information on all cancers, and available resources, as well as information about American Cancer Society programs and services.

Cancer Survivors Network is a telephone based (1-887-333-4673) and web-based (www.cancer.org) support service created by and for cancer survivors, their families, caregivers, and friends. It is designed to address their need to connect with others.

Camp Scholarship Program provides youth cancer survivors (ages 3-17) the opportunity to attend one camp of their choice per year. Maximum annual tuition scholarship is $500.

College Scholarship Program gives cancer survivors (must have been diagnosed before age 21) the opportunity to pursue a college degree from an accredited two or four-year university, community college, or vocational/technical school (also available for graduate students). Maximum $1,000 per year.

Hope Lodge is a “home-away-from-home” for cancer patients and their caregivers. It provides free lodging and a supportive environment while cancer patients are undergoing outpatient treatment.

I Can Cope is an education program about living with cancer. Sessions offer free classes led by health care providers, social workers, and others qualified to share information with patients and family members about cancer and its treatment.

Man to Man is a program that offers “one on one” and group support to men facing prostate cancer.

Look Good, Feel Better teaches cancer patients techniques to address the cosmetic side effects of treatment.

Reach to Recovery is a program in which trained volunteer breast cancer survivors provide peer-to-peer support and information to others experiencing or concerned about breast cancer.

Road to Recovery is a program where volunteers drive cancer patients to treatment.

Tell a Friend is an American Cancer Society prevention program conducted among friends, family, and acquaintances. Trained volunteer callers contact at least 5 women they know and encourage them to have a mammogram. This strategy is called “peer counseling” and has been tested and proven successful.

Collaborative Campaigns offer communities an opportunity to work as a team to meet ACS 2015 goals. Campaigns educate constituents on cancer prevention and early detection strategies increasing awareness of site-specific focus such as: Prostate, Breast, Tobacco Use Prevention, Skin, Colon, and Nutrition.
Let’s Talk About It is a community-based prostate awareness and education program for African American men developed collaboratively by the American Society and 100 Black men of America, Inc.

Tobacco Use Prevention helps groups organize and implement effective community advocacy projects. Efforts focus on prevention and advocacy for all ages directing smokers to www.cancer.org and 1-800-ACS-2345.

Relay for Life, the American Cancer Society’s signature activity, is an opportunity to create awareness about American Cancer Society programs, and to have participants engage in interactive cancer control activities. Relay for Life is also a venue for recruiting volunteers and advocates who can help shape public policy regarding cancer related issues.

Advocacy - ACS Action Network – As a grassroots advocate, you will help the American Cancer Society shape public policy regarding cancer related matters and the rights of cancer patients at the community, state, and national levels of government.

Grants - Community Development Grants are designed to financially support community projects that are consistent with American Cancer Society Southeast Division’s cancer control objectives. Grants are awarded in the amounts of $1,000 to $10,000 after review and approval by a team comprised of volunteers and staff.

Cattle Barron Ball is a major fundraiser planned for the Floyd Country United ACS. It will be at the Zion Farms on October 25, 2003.

Health Initiative for Men aids African American males of the community with free screenings (Diabetes, Cholesterol, Skin Cancer, Vision/Hearing, height/weight, Blood Pressure and Prostate Cancer). They also can learn more about domestic violence, substance abuse, physical fitness, depression, nutrition, anxiety, and STDS.

Georgia’s Breast and Cervical Cancer Program provides breast and cervical cancer screening at no cost or low cost to low income, uninsured, and under-insured women ages 40-64.

Women’s Health Program includes the Family Planning Program (Title X). Women receive many services including screening for some cancers.

Community Care Service Program helps Georgians continue to live at home in the community. CCSP may offer one service or a combination of services to best meet the needs of each person.

Tobacco Use Prevention Program coordinates strategy in tobacco use prevention and control. It provides assistance on policy development, and serves as a resource center for tobacco issues.

Prevention Initiative is the Public Health initiative to prevent illness through regular checkups, screenings, etc. at the County Health Deparments.

Nutritional Service/WIC provides proper nutrition which helps prevent birth defects and promotes healthy growth. Participants are also referred to appropriate public health and other health services as well as to nutritional, health, and breast-feeding education and assistance.

Fit to Eat is a partnership between local restaurants, the University of Georgia and the County Health Department. The goal is to assure you that you can eat out in an environment that is free from second hand tobacco smoke and enjoy tasty food items that have 30% or less of their calories from fat.

Northwest Georgia Interfaith Health Coalition builds a cohesive and cooperative effort of service between the Health and Faith Communities. This coalitions works to ensure that health, wellness and spiritual services are made known to the people of northwest Georgia by fostering collaborative efforts between all health agencies and the faith community, while advocating a healthy body, mind and spirit for all.

Georgia Cancer State Aid Program funds diagnosis and treatment for medically needy cancer patients. This program provides inpatient and outpatient cancer treatment at participating hospitals and free standing cancer centers. It also provides cancer-related drugs by approved private pharmacies.

Women’s Health Medicaid Program provides full coverage Medicaid for eligible women under 65 who have breast and/or cervical cancer and need treatment.

Bosom Buddies of Georgia, Inc. is a non-profit organization that supports women who have had or are currently living with breast cancer.

Northern Region Cancer Registry is a population based cancer registry serving the public health needs of the citizens of north Georgia.

Camp Bluebird offers a unique experience for adult cancer patients. Campers must be 18 or older and have been diagnosed with cancer.

Hospital-Based Prostate Screenings are annual prostate cancer screenings sponsored by all Regional Hospitals. These screening are free to public.

Hospital-Based Breast Screenings and Mammograms are offered by all Regional Hospitals. Both are free to the public.

Hospital-Based Skin Cancer Screenings are annual skin cancer screening sponsored by Floyd Medical Center, Hamilton Medical Center, and Redmond Regional Medical Center. Screenings are free to the Public.

Hospital- Based Cancer Support Groups are offered by all Regional Hospitals. These groups are free to the Public.

Hospital- Based Bereavement Counseling is offered by all Regional Hospitals. Counseling is free to public.

Hospital Based Smoking Cessation Classes are offered by Hamilton Medical Center, Hutcheson Medical Center and Redmond Regional Medical Center. Classes are free to the Public.

Hospice-Based Bereavement Counseling is offered by all Regional Hospices. Counseling is free to the Public.

Paint the Town Pink Breast Cancer Awareness Program is offered By Floyd Medical Center each Year and includes screenings and mammograms. This program is free to the Public.

Phoenix Support Group assists in social re-entry for the loved ones of former hospice patients. This group reintroduces persons to social functioning, and it is a free program.

Industry Health Fairs are offered by all Regional Hospitals and are free to the Public.

Hospital-Based Health Mobiles are offered by Gordon Hospital, Hamilton Medical Center, and Hutcheson Medical Center. These mobiles are free to Public and are utilized for Health Fairs and Industry screenings as well.

Logisticare Transportation Services will transport patients anywhere in the region. Medicaid and Private Pay are accepted.

Latino Health Fair is sponsored by area physicians and Hamilton Medical Center for Whitfield County each year in the Fall for the Latino Community.

Angel Transportation will transport patients in Catoosa, Chattooga, Dade, Gordon, Murray, Walker and Whitfield Counties. Medicaid is accepted.

Integrative Oncology Support Group is sponsored by the Regional Radiation Oncology Center at Rome. This group is free to cancer patients and their families.

“End of Life Decision: Your Final Gift” is a seminar sponsored by Floyd Medical Center at the FMC Health Resource Center at Mt. Berry Square Mall.

“Wellness First” Series are seminars offered to help people learn to eat healthier. This series is presented by the University of Georgia, Department of Foods & Nutrition, from May through July.

Soup Kitchen Health Fair/ Cancer Screenings will take place in Nov. 2003. A group of Floyd County churches which sponsor soup kitchens each month are collaborating to offer cancer screenings to the low income, uninsured and under-insured who frequent the soup kitchens. In conjunction, the churches are sponsoring blanket and coat drives to give to this population for completing the screening process.

Cancer Well-Fit Program is sponsored by Harbin Clinic and the Southeastern Cardiovascular Center. The program is intended to commence at the time of cancer diagnosis and continue throughout all phases of recovery. This program offers patients the opportunity to return to their fullest physical, psychological, nutritional and emotional potential.

“Surviving To Share” Program is sponsored by Redmond Regional Medical Center and Barnsley Gardens Resort. Individuals who have survived breast cancer are encouraged to submit their stories. Three winners will receive weekends for two at Barnsley Gardens Resort.
